2011-2012 Senior Basketball Leagues / Clubs |
As we enter the fall of the year, it is once again a reminder to start basketball in the province. Following are a number of forms, which require completion prior to the start of the basketball season. All forms are clearly marked and must be fully completed by members of each association.
League Affiliation Form: This form is to be completed by the executive members of your association. It enables our association to record all leagues and their executive members across the province. The cost for league affiliation is $100.00, and it provides your association with voting privileges and 50% of costs incurred for one representative to travel to our Annual General Meeting each June.
Player Registration Form: This form is to be fully completed by each participant on each team in your league. Please ensure addresses are fully completed including the postal codes, P.O. Boxes, etc. This is imperative for insurance requirements. During the coming season all names without complete addresses will be erased from our database and will no longer be eligible for insurance. I have enclosed a number of these sheets based on last year’s teams. Individual registration fee is $25.00 per player, executive, coach, etc. |
